What causes error 1095B-010-01, 1095C-010-01, Shared-008? |
Each of these errors indicates a specific Name/TIN validation error. More specifically,
Error 1095B-010 indicates that the Name/TIN combination for a Responsible Individual did not match IRS records.
Error 1095C-010 indicates that the Name/TIN combination for an Employee did not match IRS records.
A Shared-008 error indicates that the Name/TIN combination for a Covered Individual did not match IRS records.
Pursuant to IRS, a Name/TIN combination mismatch can happen for a variety of reasons:
The social security number reported is incorrect;
The last name reported is misspelled;
A maiden name is reported when the married name is registered with SSA;
A married name is reported when the maiden name is registered with SSA;
Hispanics may sometimes have two last names. |

I received a Shared-008 error. How do I find the Covered Individual? |
Although the IRS Acknowledgement does not identify the Covered Individual by name, it does provide a useful reference pointer to the Covered Individual, as listed on the 1095 containing the Covered Individual. The reference pointer is located in the brackets [] following the CoveredIndividualGrp, i.e.
CoveredIndividualGrp[reference pointer]
Since Account Ability's XREF file identifies the Employee (Responsible Individual) belonging to the 1095, you need only locate the 1095 and refer to the referenced Covered Individual.
Example: The second Covered Individual is referenced in the following Shared-008 error,
/Form109495TransmittalUpstream/Form1094UpstreamDetail/Form1095UpstreamDetail/CoveredIndividualGrp[2]SSN |

Why am I getting the error "Self-Insured coverage conflict" when trying to create my 1095-C transmittal? |
IRS business rules require that you check the box Employer provided self-insured coverage if, and only if, there is at least one covered individual. If the employee and all of his/her dependents opted out of the self-insured plan then you must uncheck the Employer provided self-insured coverage box.
Consequently, a "Self-insured coverage conflict" error will occur if either,
The employee and his/her dependents opted out, but the Employer provided self-insured coverage box is checked, or
The employee and his/her dependents opted in, but the Employer provided self-insured coverage box is not checked.
FYI: The 1095-C in error will be displayed when Account Ability stops on this error.
TIP: Adjust Self-Insured Indicators on the Tools menu of Form 1095-C will correct this error on all 1095-C forms. |
